Universal Health Services SOCIAL WORKER / COUNSELOR in YOUNGSTOWN, Ohio

Responsibilities

The Case Manager provides services to patients and families acting as a liaison between treatment team, physician, referral source, community agencies, outside therapists, and third party reviewers. The Case Manager maintains the integrity of the medical record of assigned patients; assist in the preparation of the master treatment plan; initiates discharge planning; contacts discharged patients within established time frames for follow-up and assists in completing intake screenings and makes referrals. Performs other related duties as assigned by the Supervisor.

Qualifications

Education: Bachelors Degree in Social Work or related degree in behavioral science. Social Services degree preferred. Knowledge of developmental and life stage of and psychiatric problems peculiar to younger childhood (ages 3-7), middle childhood (ages 8-12), and adolescent populations is highly preferred. Knowledge of Belmont Pines Hospital policies and procedures.

Licensure : None required.

Experience: Previous work experience in a mental healthcare facility is preferred.

Skills/Traits: Ability to communicate information in a positive manner with all levels of people, and age groups. Must be able to maintain professionalism in dealing with referral sources within the community, patients, families, physicians, and staff members. Able to maintain confidentially of information contained in the medical record of assigned patients. Able to act in a mature manner in situations that calls for diplomacy and tact. Should possess a command of the English language with good verbal and written skills. Must be organized and possess good administrative skills.

Competence: The individual must demonstrate competence by:

  • Ability to obtain and interpret information.

  • Understanding of the biological and sociocultural dimensions influencing mental health.

  • A general understanding of the range of treatment needed by dependent patients.

  • Ability to work with adolescent and child populations as demonstrated through educational accomplishment, prior work experience or job training specifically related to the population of the unit.

  • Ability to maintain certification in CRP and physical crisis intervention (PCI).

    This individual must possess excellent verbal and written communication skills. Diplomacy and discretion are essential, as well as excellent organizational skills and the ability to prioritize multiple projects. Professional demeanor and appearance required.
