Responsible for analyzing, assessing, and performing chart corrections and identity investigations to maintain the integrity of each patient record within the enterprise master patient index, ensuring one record per patient is preserved. Works with close supervision and/or within established policies/practices and guidelines.

Principal Accountabilities:

• Analyzes and merges potential chart duplications and executes chart correction processes as identified seeking assistance when necessary.

• Places chart advisories as appropriate. Assesses impact to Care Everywhere, MyChart, and other downstream systems.

• Appropriately directs requests for assistance via Cherwell Service Management, the Epic AHS Help Desk, and email.

• Works assigned HIM Data Integrity Services work queues and reports.

• Communicates and collaborates with internal and external contacts to resolve complex chart issues and follows up according to department procedures.

• Assists in identification and communication for areas of non-compliance with established policies and procedures to decrease the creation of duplicate medical records, overlays, and other data integrity issues.

• Performs other duties as assigned.

Qualifications

• Bachelors, Associate degree, or equivalent experience required.

• Skills/knowledge/abilities typically gained through 0-2 years of related experience required.

• Epic experience preferred

• RHIT, RHIA preferred
