Lasante Health Center is seeking a Care Coordinator to join our team. The ideal candidate will monitor our patients’ medical treatment and facilitate communication with healthcare providers to guarantee access to high-quality medical care.

Responsibilities:

• Regularly monitor and manage a designated group of patients with chronic medical conditions.

• Enroll patients in different medical programs

• Engage in consultations with patients and their family members to address health concerns.

• Provide comprehensive education to patients regarding their medical conditions, medications, and specific care protocols.

• Assist patients in scheduling appointments

• Complete other tasks as directed by supervisors.

  Qualifications:

• Customer service experience

• Computer skills

• Ability to handle multiple tasks concurrently

• Care coordination experience preferred
